Medication support

Reminders, help and, where agreed, giving medicines safely, with every dose recorded.

Medicines need to be taken at the right time, in the right way. We agree the level of support someone needs during the assessment and write it into their care plan.

Levels of support

  • Reminding someone to take their medicines when they manage them independently
  • Assisting, for example opening packaging or pouring a drink
  • Giving medicines where this has been assessed and agreed
  • Applying prescribed creams, and putting in eye drops
  • Collecting prescriptions from the pharmacy

Keeping it safe

Carers complete medication training and are checked as competent before they support anyone with medicines. Every dose is recorded on a medication administration record, and we contact the GP, pharmacist or family if something doesn’t look right, such as a missed dose or a change in how someone is.
